Evaluation form: allow to combine different answer choices

I am glad that indico improvements are being made. Thanks.

Since there many tutorials that CMS experiment holds, we make feedback forms to evaluate a tutorial. RIght now on indico, one cannot combine different answer choices in the same question.

For example one cannot combine "check box" with "text area". This feature is available in commercial software surveymonkey and is very useful. It would be nice to have this flexible feature.
