Full author, co-author and underlined presenter changes to book of abstract output

Description
The original book of abstract only listed the presenter.
The supplied diff provides author and co-author, with the presenter underlined.
Added a authStyle style.
Diff patch attached.

It would be nice if you could attach a unified diff (use the -u argument when creating the diff)

This has already been fixed. The book of abstracts has meanwhile undergone a lot fo changes and it now shows only the authors, not the speakers.

I meant to add: If there is a strong reason to leave the existing book of abstracts alone (generated PDF), please could someone look into providing an update to the previous menu, to add this patch as a new PDF generation option:
