EVO: display page improvements

The event manager should have the following options when creating an EVO booking:

1. Display the booking's password (for Koala) in the display page (this is task #173)
2. Displaying the Phone Bridge id (we need to ask the EVO team to include this info in the returned meeting information).
3. Displaying the Phone Bridge password (most probably EVO needs to also send to us, needs confirmation)
4. Displaying a link to EVO phone bridge numbers.

Option 1 should be a checkbox in the advanced tab, unchecked by default.
Option 2 should be a checkbox in the advanced tab, checked by default. In the sentence after the checkbox there should be a select box to choose option 3 (not showing password by default).
Option 4 should be a checkbox in the advanced tab, checked by default.

Changing the priority to high since it seems more users are in need of knowing the phone bridge id / phone bridge password. The phone bridge password in particular cannot be known at the moment by the users, at all, so there's no workaround other than EVO sending to Indico this information.
