Improve evaluation form

Make the evaluation form more usable and fix some small issues.

User feedback

• would appreciate a way to add introductory text (i.e. simple readonly non-form elements, such as a text paragraph or headings or separators)
  • this could also be used to implement logical subsections. If not, such subsections would be a nice addition

• Some way to convert each element of the programme/timeline into a (set of) feedback field(s) could save a lot of time. There would have to be a way to filter/remove unwanted entries ("lecture quality" of "coffee break" does not make much sense..). Something like
  • create a template/subsection with one or more questions - typical would be "please rate technical content" (radiobox), "please rate presentation" (radiobox), "any comments?" (textbox),
  • then instantiate these questions for each of the contributions
  • (then present list and allow to remove superfluous ones)

• It is unclear (to me) at which moment the "start of evaluation" mail will be sent (or has been sent?), if an "open" evaluation gets added to an existing event. Perhaps some message like "start mail sent at XXYYZZ" or "start mail will be sent once ActionX happens" could be added to the "setup" page, or at least to the "Log" page?

• (cosmetic: deleting a question gives a popup warning about deleting existing answers (OK!), but mentions something like a referee role(? sorry, forgot exact wording, no screenshot) which is not immediately obvious (i.e. neither submitter nor surveyor))
